Completing Repair Form from the Carrsmith Service Department If you drop off your car after hours, please see blank forms in the holder to the right side of the office door. Is your car being towed in and you would prefer not to have to ride in to the shop with the tow truck driver? Print out the form at home while waiting for the tow truck, fill it out and leave it in the car for us. We'll call you after we've checked it out. If you did not notice the blank forms when you dropped off your car after hours, this online copy may help you save an extra trip to the shop. Please fill out the form per instructions below.
Check
one of the 3 boxes on lower left of form. Option #1: Written Estimate. Check the first box if you want a written estimate. You will automatically be liable for up to $100 and from that point forward all authorizations will be in writing- no verbal authorizations will be permitted. Option #2: Verbal Estimate. Check the second box if you want a verbal estimate. Write an amount in the a blank to the right that you would allow us to got up to but NOT exceed without your permission. Any changes or additions may be carried out conveniently by phone or in person and will be documented with a date and time stamp on your final invoice Option #3: No Estimate. Check the third box if you want no estimate. Checking the third box is your direction for us to simply do what is needed without any monetary limit or advance communication.
Filling out the form Vehicle Information: Please just full in the license plate (tag) number, year, model and color of your vehicle. If you are not near the vehicle or a copy of the registration, don't worry about the Vehicle Identification Number (VIN). We can get that information along with the engine size, transmission type, number of gears and other technical details.. Client Information: Fill out this area completely including complete name, complete address and apartment number, if any. Please fill in your phone numbers, and other items in this area. Description of Concerns: Please read the Communicating with your Service Advisor link to the left. Please write a basic description of your concern. Please write what the car is not doing that you would like it to do or, what the car is doing you would prefer it not do. It's helpful for us to know how long you have had the concern and what lead up to it. If you need more room use the back of the form. If you think you know what's wrong with it and want to write that, no problem. But we still need to know what the car is not doing that you would like it to do or, what the car is doing you would prefer it not do Read and Sign the Form. |
