Completing Repair Form

                              from the Carrsmith Service Department

When to use this form: 

  • If you drop off your car after hours, before coming to the shop you can print this form (see the 'Print Blank Repair Form' link to the left, and instructions below) and fill it out in the comfort of your home, or office. When you get to the shop, just drop the completed form and your car keys through the mail slot in the office door. If you forgot to print the form at home, don't worry. There are blank forms in the holder to the right side of the office door that has the mail slot.

  • Is your car being towed to the shop during our normal operating hours? Would prefer not to have to ride in to the shop with the tow truck driver? While waiting for the tow truck driver, print out the form (see instructions below) at home, fill it out, and leave it on the front seat of your car for us. If it's after hours just have the tow truck driver drop your key through the mail slot in the office door. We'll have all your contact information on your completed form so we can call you after we've checked it out for you. AAA MEMBERS, PLEASE NOTE: AAA tow policy requires someone to be with the vehicle when it is picked up, and they require someone to be on the receiving end, as well. So, depending on the situation, when having your car towed to the shop by AAA after hours they may ask you to meet the tow truck at the shop when they get there to drop it off. 

  • Did not leave us your completed form when you dropped off your car after hours, or had your car towed to the shop after hours? The 'Print Blank Repair Form' link on the upper left side of this page will help you save an extra trip to the shop (see instructions below).

Instructions for the Print Blank Repair Form link

  • Please go to the Print Blank Repair Form link at the top of the blue area to the left.

  • Please print the form.

  • Read the form.

  • Fill out the form.

  • Check one of the 3 boxes at the lower left side of the form (see Explanation of the 3 boxes, below).  

  • Sign the form at two (2) place on the bottom.

  • Scan the completed form (if you can, please save as a pdf file) and email the form back to lisacarrsmith@bellsouth.net

  • Or, fax the form to 352-378-8649

  • Or, take a picture of it with your cell phone and email it back.

    Option #1: Written Estimate. With this option, the customer will automatically be liable for $100. This option will require all customer repair decisions to made in writing. It will require the customer to sign all their written repair decisions. It will require the customer to deliver their written and signed repair decisions to the shop. From that point forward, the customer will not be permitted to authorize any repairs, either by phone or in person.

    Option #2: Verbal Estimate. Check the second box if you are ok with authorizing your repairs verbally, either in person or by phone. Write an amount in the a blank to the right of your checkmark that you would permit us to got up to BUT NOT EXCEED, without your permission. Any changes or additions may be carried out conveniently by phone or in person, and will be documented with a date and time stamp of your decision on your final invoice

   Option #3: No Estimate. Check the third box if you want no estimate. Checking the third box is your direction for us to simply do what is needed without any monetary limit, or advance communication.

 

Filling out the form

Vehicle Information: Please just full in the license plate (tag) number, year, model and color of your vehicle. If you are not near the vehicle or a copy of the registration, don't worry about the Vehicle Identification Number (VIN). We can get that information along with the engine size, transmission type, number of gears and other technical details..

Client Information: Fill out this area completely including complete name, complete address and apartment number, email address, etc. Please fill in your phone numbers, and other items in this area.

Description of Concerns: Please read the Communicating with your Service Advisor link to the left. Please write a basic description of your concern. Please write your description of what the car is not doing that you would like it to do, or what the car is doing you that would prefer it would not do. It's helpful for us to know how long you have had the concern and what lead up to it. If you need more room use the back of the form. If  you think you may know what's wrong with it and want to write that in addition, no problem. But we still need to know what the car is not doing that you would like it to do or, what the car is doing you would prefer it not do
